CD138 is a member of the trans-membrane heparin sulphate proteoglycan family involved in many cellular functions including cell proliferation, cell migration, cell signalling, cytoskeletal organisation and mediates both cell-cell and cell-extracellular matrix interactions. Additionally, via its heparin sulphate chains, it binds a wide range of bioactive molecules that regulate cell behaviors important in normal and pathological processes. CD138 can exist in a membrane bound form and in a soluble secreted form.
Membrane Bound CD138
Under normal healthy conditions CD138 is expressed on mature plasma cells, early pre-cursor B-cells and various types of epithelial cells. CD138 is also identified in practically all cases of plasma cell malignancies. Therefore, CD138 identification can act as a specific marker for plasma cells and plasma cell differentiation and its identification is a key tool to separate and isolate plasma cells and aid the diagnosis of haematological malignancies.

Soluble CD138
CD138 shedding can be induced by several inflammatory factors and is evident under certain pathological conditions. The Shed CD138 remains an effector molecule capable of binding the same ligands as the membrane bound form.
Studies have shown the serum levels of soluble CD138 detectable by ELISA assay can be used to aid prognosis in certain forms of multiple myeloma. In addition, using animal models, a clear role in the development of multiple disease states has been identified for CD138.
